People with a chronic illness make most of the decisions about their health outside of the medical office. Our Chronic Disease Management Program helps patients with a chronic illness manage their own care.

The important parts of our Disease Management program are:

• An ongoing relationship with one health care provider
• Reminder letters and phone calls when care or tests are due
• Support for self-care management; making goals and updating progress
• Coordination with specialists and other parts of the health care system
• Assistance with free or lower cost medications
• Reporting lab results to patients by mail
• In-office diabetes and nutrition education
• Group nutrition and self-management classes
• Home monitoring of blood sugar and blood pressure
• Linkages to community resources
