At FamilyCare, we not only treat our patients' medical needs, we try to help people take care of the other needs in their lives so that they can focus on their health. Social Workers are valuable members of our health care team.
FamilyCare Social Workers:
- Connect our patients to community resources for food, shelter, clothing, transportation, and many other needs
- Provide emotional support and counseling, in a confidential atmosphere
- Help people handle problems and crises
- Work as part of the health care team to help patients learn about and manage their illnesses
- Work closely with our patients with chronic illnesses such as diabetes, cardiovascular disease and chronic pain
- Coordinate our educational classes and events for the community
